CRANE brings together the experience of three rural regions with increasing levels of depopulation and a common and urgent need for transformation-Region Västerbotten in Sweden, Extremadura in Spain, and Agder in Norway. Norwegian, Spanish, Swedish and Danish partners complement the team with their multidisciplinary backgrounds and experience in PCP projects.
PROJECT / OBJECTIVES DESCRIPTION: CRANE is a PRE-COMMERCIAL PROCUREMENT European project that aims to develop an integrated selfcare model to improve chronic patient’s wellbeing.
The EU's population is ageing, and at the same time, those living with disabilities and chronic diseases are on the rise. In rural areas, individuals living with a disability or chronic disease are less likely to have access to high-quality health services and programmes compared to those in urban areas. The EU-funded CRANE project, focusing on the comprehensive treatment of chronic diseases in rural areas, aims to build a system for increased person-centred care and nursing whereby more than 80 % of chronic patients move to self-care. The project will propose a model which is supported by two pillars: healthcare from home and the ecosystem.
OPEN MARKET CONSULTATION (OMC): The OMC is scheduled to start on March 2022 and to end by June 2022, and the call for tenders publication is expected on August 2022.
